Dentist Needed to provide preventive dental services for dual eligible, frail elderly and disabled patients in our Program of All Inclusive Care for the Elderly (PACE). I will be providing primary care and comprehensive and coordinated care management with the goal to maintain at-risk patients in their homes and prevent institutionalization. As such, preventive health care including dental care is central to the organization and delivery of our suite of health care services. We have been successful in contracting for podiatry, optometry and psychiatry services to be provided on site monthly or bi-monthly at our center’s clinic, at the address below. PACE program participants , as is the case with all vulnerable patients, are less stressed with maintenance of continuity in care and the security of familiar environments of care.
